compassionate and dedicated Palliative and Population Health Nurse
Navigator (NN). In this role, you will be responsible for providing
comprehensive care coordination and navigation services to patients
with serious illnesses, ensuring they receive the highest quality
of care throughout their healthcare journey with illness and
loss.

Office Location: Largo, MD
Coverage Area: Prince George's and Charles Counties
Status: Fulltime, Monday - Friday
Salary: Competitive, commensurate with experience, education and
credentialing
Main Responsibilities:
Respond promptly to clinical inquiries, determine needs and next
steps, take timely action and document interactions in EMR.
Provide in-home and telehealth assessments, document findings and
care plans, and provide ongoing care navigation/coordination for
patients and families.
Educate patients, families, team members, and referral sources
around agency programs, including eligibility criteria and the
referral and admission process.
Educate and enroll patients into hospice services.
Complete necessary paperwork and administrative documentation for
assigned tasks and insurance verifications.
Coordinate with clinical and administrative team members to ensure
timely completion and reporting of program quality metrics.
Proactively coordinate around patient needs/questions with
referring medical practices to ensure highest quality patient
care.
Monitor the EMR and other program channels to expedite patient
needs.
Spot inaccurate and incomplete information in a patient's insurance
record and seek out whatever information is needed to complete the
record.
Always represent the agency in the highest light with patients,
families, and referral sources.
Provide education to internal and external audiences when
requested.

Your Qualifications:
Active State of Maryland or Compact RN license in good standing
required.
2-3 years of nursing experience in medical/surgical, critical care,
home care, hospice or palliative care setting
Associate's degree in nursing or related field, BSN preferred.
Certification in Hospice and Palliative Care Nursing (CHPN)
preferred and required within two years of employment.
Excellent nursing assessment and verbal presentation skills
required.
Enthusiasm for nursing and passion for caring for patients with
serious illness are essential.
Time management and high-quality organizational skills
required.
Proficient in Microsoft Office Products and EMRs.
Demonstrated ability to simultaneously monitor multiple
communication channels, handle multiple ongoing projects while
meeting deadlines, and foster beneficial relationships with
internal and external colleagues/stakeholders.
Solve problems creatively and efficiently.
Current Maryland driver's license and clean driving record.
Possess or access to reliable transportation.
