  • FACILITY RADIUS RULE IS WAIVED FOR THIS POSITION
  • FLU VACCINE: Davis will consider medical exemptions. Travelers MUST provide a signed doctor's note from their physician AND complete Davis' medical exemption request form. Davis is NOT considering religious exemption requests
