Summary:We are seeking a dedicated and skilled RN Transfer
Coordinator to join our Patient Logistics Center team. This role is
pivotal in managing the transfer process for patients from
referring hospitals, emergency departments, and other facilities
ensuring a smooth and efficient transition into our care.
-Responsibilities:
- Coordinate the transfer of patients to and from the hospital,
working closely with physicians, nurses, and other healthcare
professionals.
- Assess patient information and clinical data to facilitate
appropriate bed assignments and prioritize transfers based on
clinical needs.
- Communicate effectively with referring and receiving
departments to ensure timely and efficient patient transfers.
- Maintain accurate records of all transfer requests and patient
movements within the hospital's electronic health record
system.
- Participate in the development and implementation of policies
and procedures to improve the efficiency of the Patient Logistics
Center.
- Provide exceptional customer service to patients, families, and

Qualifications
- Current RN license in DC, or eligible to obtain.
- BSN preferred; ASN or nursing diploma considered with relevant
experience.
- Minimum of 2 years of nursing experience, preferably in
emergency, critical care, or a similar high-acuity setting.
- Excellent communication, organizational, and problem-solving
skills.
- Demonstrated ability to work effectively in a fast-paced
environment.
- Proficiency in electronic health record systems and basic
computer applications.
