Role:
The Registered Nurse (RN) Case Manager provides, coordinates,
teaches and supervises the nursing
care of Hospice patients and families/caregivers. Working in
collaboration with other members of the
interdisciplinary team/group, the RN Case Manager plans and manages
the nursing component of the hospice caseload. The RN Case Manager
makes initial and ongoing nursing assessments, judgments and
treatment decisions based on patient and family/caregiver needs and
wishes and in accordance with her/his professional skills.
Supervisory Responsibilities
The RN Case Manager is responsible for supervising LPNs, CNAs, and
Patient Care Volunteers.
Experience Requirements
The RN Case Manager must have at least one year of staff nurse
experience in a comparable
setting/unit. Hospice and/or palliative care experience is strongly
preferred.
Education Requirements
The RN Case Manager must have a degree in Nursing from an
accredited nursing program. A BSN
degree is preferred.
Required Certifications and/or Licenses
The RN must have licensure from the governing body for practice for
the District of
Columbia, State of Maryland, and/or the Commonwealth of Virginia,
based on the
state/jurisdiction of practice; or licensure from compact state.
Healthcare Provider BLS (CPR)
is required to practice in the State of Maryland and the District
of Columbia. A CHPN
certification is a plus.

Technological Skills
The ability to input and manage data in patient health information
systems and demonstrate proficiency in Microsoft Office and
internet browsers.
Reasoning Ability
The ability to apply common sense understanding to carry out
instructions furnished in written or oral form and deal with
problems involving several concrete variables in standardized
situations.

Work Environment
The work environment characteristics are representative of those an
employee would normally
encounter while performing the essential functions of this job in
an office. Reasonable

computers, printers, copy/fax machines and telephones. The noise
level in the work environment is usually light to moderate. The
environmental conditions are that of what is to be expected in a
temperature-controlled building.
